Support coordinators, plan managers, health professionals, families and participants can all refer to Esteem Mentoring. Complete the short form below and we will contact you to talk through the participant's needs and our availability. We always speak with the participant before any support starts.

What happens next

  1. We contact you, usually by phone, to talk about the referral.
  2. We check suitability and availability, and let you know honestly if we can help.
  3. We arrange to meet the participant, and anyone they would like to involve.
  4. If everyone is happy to go ahead, we move through intake, planning and worker matching. See Getting Started for the full process.
