NDIS community access and participation support

Community access support helps you get out and take part in the activities, places and relationships that matter to you. Esteem Mentoring provides one-to-one community participation support for NDIS participants across Moreton Bay and Brisbane, planned around your interests and the pace that suits you.

More confidence and connection in your community

Community participation should look like your life, not a generic list of outings. For one person it is footy training on Tuesday nights and a coffee with mates afterwards. For another it is getting to the library, joining a craft group or finally feeling comfortable at the shopping centre.

We support you to do those things in a way that feels safe and enjoyable, and that has a purpose. Over time, the goal is usually to need a little less help with each activity, and we plan with that in mind.

Support may include

  • attending social, sporting, recreational and cultural activities
  • shopping and everyday errands
  • getting to appointments and planned outings
  • joining clubs, classes, volunteering or community groups
  • exploring study, work experience or employment goals
  • building confidence with public transport (see Transport and Travel Support)
  • practising communication and social skills in real settings
  • developing routines that make community life easier

A mentoring approach, not just a lift and a chaperone

Our workers do more than accompany you. They encourage you to make choices, work through problems and build skills, while giving you the level of help you want and need on the day. If you want to order your own coffee, they will step back. If a busy place gets too much, they will help you take a break and decide what happens next.

That approach comes from our mentoring background, and it is why many participants use community access and Mentoring and Life Skills together.

Planning and safety

Before support begins we talk about your goals, interests and communication needs, plus anything that could make an activity harder or riskier. We write that down so every worker who supports you is on the same page.

We review the plan when your circumstances change or when you ask us to. You have the right to try new things and take reasonable risks, and we will support you to do that safely.

Who this support suits

Community access suits adults aged 18 to 65 who want more independence or connection outside the home. We support people with acquired brain injury, autism, intellectual disability, psychosocial disability and physical or sensory disability.

Community access questions

What is NDIS community access?

Community access, also called social and community participation, is support that helps a participant take part in activities outside the home. It can include social outings, sport, hobbies, appointments, volunteering and building the skills to do these things more independently.

Can my support worker drive me to activities?

Yes, where it is part of your agreed support. Our workers can transport you to and from activities, and the kilometres travelled are usually charged as activity-based transport. We will explain the costs before support starts.

Can I choose what we do each week?

Yes. You choose the activities. We help you plan them around your goals and budget, and we will suggest ideas if you want some.

Can I have the same worker each time?

That is what we aim for. We match workers carefully and keep the team around you small, so you see familiar faces. We will let you know in advance if your regular worker is unavailable.
