What information we collect

We collect information we need to provide safe and appropriate supports. This can include your name, contact details, date of birth, NDIS number and plan details, emergency contacts, health and disability information, support plans, behaviour support information, cultural and communication preferences, and notes about the supports we provide.

Through our website we collect what you enter into our forms, plus basic analytics about how the site is used (see Cookies and analytics).

How we collect it

Mostly from you or your nominated representative. With your consent we may also collect information from your support coordinator, plan manager, health professionals or other providers.

How we use it

To assess whether we can support you, plan and deliver supports, match workers, keep you safe, manage invoicing and funding claims, meet our legal and NDIS obligations, respond to feedback and complaints, and improve our services.

When we share it

We only share your information with your consent, or where the law permits or requires it. This can include your plan manager, support coordinator or other providers involved in your support, the NDIA or the NDIS Quality and Safeguards Commission, and emergency services if there is a serious risk to someone's health or safety. We do not sell personal information.

How we store and protect it

Records are stored securely, in electronic systems with access controls and in locked storage for any paper records. Only authorised staff can access participant information, and only when they need it for their role. We keep records for the periods required by law and then destroy them securely. Website enquiries are deleted after 24 months unless they become part of a participant record.

Accessing and correcting your information

You can ask to see the information we hold about you and ask us to correct it. Contact us on 07 3882 1863 or info@esteemmentoring.com.au. We will respond within 30 days.

Our website uses cookies for analytics (Google Analytics) only if you accept them in the cookie banner. These help us understand which pages are useful. We also use Google reCAPTCHA to protect our forms from spam, which is subject to Google's Privacy Policy and Terms of Service.

Please do not include detailed medical, behavioural or other sensitive information in our website forms. We will arrange a secure way to collect that information if it is needed.
